Who De Boomgaard often suits

  • Older people living at home with little to do or little contact during the day
  • People with early memory problems who benefit from rhythm and familiar faces
  • Situations where a partner or child who provides care could do with a break

Not sure?

Call the coordinator. We are happy to think along, even if De Boomgaard may not be the best place. A trial day is always possible, even before funding is arranged.

Wmo or Wlz? Here is how it works, in plain words

A short explanation for clients and families. Feel free to pass this page on.

Wmo

Through the municipality

Most people who live at home receive day care through their municipality, under the Wmo (Social Support Act).

  1. You or the referrer contact the Wmo desk of the municipality of Vlietstad.
  2. A conversation follows, often at home: the so-called kitchen-table talk.
  3. The municipality decides and sends a written decision. You usually pay a personal contribution.
Wlz

Through the Long-term Care Act

People who need a lot of care permanently can get a Wlz assessment. Day care can then sometimes be arranged through the Wlz.

  1. The CIZ assesses whether someone gets a Wlz assessment.
  2. The care office then arranges the care, including day care.
  3. Some people choose a personal budget (pgb).

What De Boomgaard does and does not do

We do not decide whether someone is entitled to Wmo or Wlz support. The municipality and the CIZ do that. We do help: with information, a free trial day and, if the client wishes, a short report of that day for the conversation with the municipality.
