Care & support

Care after a hospital stay

Coming home after a hospital stay is a relief, but it can also feel daunting. We make sure care at home is ready from the start. We coordinate with the hospital and with you, so you know who is coming and when.

What we do

  • Handover with the hospital before you go home
  • Care from your first day home, weekends included
  • Help with medication, wound care and personal care
  • Scaling back as soon as you can manage yourself

What this does not include

  • Medical advice about your recovery: your doctor provides that

Who it is for

For anyone in Vlietstad who needs temporary care at home after a hospital stay.

How care starts

  1. The hospital, your GP or you yourself get in touch
  2. Going home today or tomorrow? Call us directly
  3. We coordinate with the hospital and with you
  4. Care from day one, then scaling back together

How is it paid for?
